Position Overview:
As a Home Support Assessor, you will play a crucial role in our clients-centred approach to care. You will conduct assessments using a standardised national assessment process that focuses on wellness.
Your responsibilities will include:
- Conducting thorough assessments of client's needs in their home environment.
- Collaborating with clients, families, and caregivers to develop customised care plans.
- Recommending appropriate services and support resources based on assessment findings.
- Monitoring and reviewing care plans to ensure they meet evolving client needs.
- Providing guidance and support to clients and caregivers on accessing community resources.
Essential Capabilities and Experience:
- Compassionate and empathetic demeanour.
- Experience in conducting client assessments and/or service delivery in a community or health related position.
- Strong interpersonal and communication skills.
- Ability to assess and respond to the unique needs of clients.
- Intermediate computer skills in Word, Excel and Outlook
- Current Australian driver's license, comprehensively insured vehicle and the ability to travel as required
Preferred Requirements:
- Aged Care experience/NDIS
- Demonstrated commitment to assisting older adults to live independently.
- Experience in Health or Human Services/Community Service Coordination.
- Demonstrated commitment to providing quality services
- Active listening skills
